Add mechanism to allow the additon of namespace-prefixed functions #606

Extension functions like format-number() (issue 605) should not be in the 
XForms (or default) 
namespace, but in their own namespace.

We therefore need to add a mechanism that allows functions to be placed into a 
specific 
namespace.
