Update to match pandoc 1.16 #32

With pandoc 1.16, links and images now have attributes -- a third argument in addition to the existing caption and target.

I think I've done all of that now. I couldn't test most of the examples, since I don't have the relevant software, but since the change is trivial.... (Famous last words.)

I bumped the version to 1.3.0 (is what you intended? or should it be 2.0.0?), and updated the README, partly by adding descriptions of missing examples.
