User input for exclusions list #16

Mustache template update for parameterized fixlet.

This PR did not go through pre-commit so it has a failing check there, which is minor.

It is also missing nfs3 and nfs4 in the default exclusions, but I will add that back.

This new change will cause the security dialog to pop up in the console due to the javascript for other operators. This is a bummer, but kind of unavoidable for custom path exclusions to work, which seems needed.
