`hideThoughExpanded`: Extra options

The main options have already been done.

Options

• [ ] By vocabulary_id (put this back in place!)
• [x] By zero record count
• [x] By non-standard
• [x] All but first occurrence -- sort of working but creates problems...need to fix
• [ ] By any value of any (low-cardinality) concepts_with_counts column (domain_id, concept_class_id, standard_concept)
• [ ] Will need to have coherent hierarchy of concepts after doing exclusions, filling in gaps where necessary
