What changes are being implemented in this Pull Request?

We realized that Coursera doesn't have an easy to find feedback form, so we are adding one to the footer.

This will automatically get incorporated on all pages upon rendering.

There's some weird capitalization inconsistencies I couldn't get rid of either and not sure if they matter.
