Help Request: Procon Mapstats Integration Fails

Please describe your problem:

Using both standalone and SMF integrated procon map stats are unable to load.  
The only thing that happens is the spinning ajax wheel below the scoreboard.

The problem:

I have copied all of my server database settings from another plugin which is 
able to connect to and access the database without problem.  They are actually 
all on the same box.  However, no matter the settings applied I can not get 
ezstats to display the stats which

server viewer v0.61 
ezStats v0.88

Hi!

With the debug mode, you can see the errors in the browsers console. In your 
case, the error is, that the SQL Table 'cmw_combinedstats.server_player' 
doesn't exist. Perhaps you've entered the wrong prefix?
