Allow setting default reviewer id in preferences #105

When switching to a review or a phase of a review, it's annoying to have to
select the correct reviewer id each time.  You should provide a preference
setting for the "default reviewer id", which a person would set to
themselves (in whatever naming pattern the group agrees on.
