Can we add a new survey ...ie when you enter a core?

Perhaps this could for the first time a member clicks on a page...and every time, until they complete it. For example for the

Biology Core

In your research do you employ the following:

1. Mammalian models (tick box -> mouse, rat, primate (tick those that apply))
2. Cells (tick box -> Fibroblasts, Immortalized cell lines (e.g. HEK293, HeLa, PC12), Primary neuronal cultures, Inducible pluripotent cell lines)
3. Nucleic acid (DNA, RNA, plasmids)
4. Antibodies and/or protein
