categories dropdown in the upload form #33

🟩❗ BUG: "categories" drop-down, in the upload form, do not match up to what categories (catids) are in phpMyAdmin, AFTER fixing other related bugs, edit approp php/html page in form.

edited the order of elements in the html to reflect order in the db.
