cant get it to work properly

SO i cant get it to work for the life of me. maybe i am not 
understanding how it works. With the new feature in sipdroid i was 
under the impression that it would setup all pbxes account for me and 
i wouldnt need a cip provider like sipgate or gizmo5. Is that right? 
if so how the hell can i make it work? when i try to dial a number it 
says my call cant be completed as dialed and i am dialing a US number 
1xxxxxxxxxx. 
a step by step guide would help a lot
