add mutations #12

Hi @jodyphelan - the new delamanid resistance conferring mutations make sense in light of their associated with the F420-dependent bio-activation pathway (discussed here).

I don't understand the odds-ratio, etc columns in your CircleCI output though? They all seem to be the same.

Ah yeah they are all the same because there are no isolates which have phenotype results for delamanid in the test dataset. I'll ammend that in the future to print out NA. Ok I'll merge in the branch to master. Thanks!
