Reorganize HTML of Record page:

• [x] Add indication that users should upload a photo of themselves (if they want to), not just any random image.
• [x] Make "Click here to upload image" input button the same size as other buttons on the page
• [x] Make room (just a div somewhere above the recorder interface) for provocation text (this will be coming from Mike or Tess)
• [x] mentioned in other issue, but: the Send Files button and Continue button are confusing, make only one of them visible at a time or combine their functionality
