Issue reported with the following settings:

Open all tabs next to the active tab and place consecutive new tabs in order
Activate the last active tab
Focus the new tab except when holding Ctrl
Shift+Click opens tabs in the same window

Confirmed. The main reason I installed ClassicTabs was to make middle-clicking a bookmark switch to the new tab. It works for links embedded in web pages, and it works for the bookmark sidebar, but it doesn't work for the bookmark toolbar.
