Here are a couple of open source Chicago projects with map dashboards that come to mind:

OpenGrid http://opengrid.io/ Chicago data portal map launched earlier this year http://chicago.opengrid.io/opengrid/ I don't love it, but better than what they had before. Repo https://github.com/Chicago/opengrid
I don't think either of these allow users to upload data, but I have a couple other examples that I'll dig up and post
