Suspect this is to do with merge filenames.

This is a regression from 2.5.1. It is to do with Sturcture tree modification on merging not liking multiply-included files. Need to:

re-adress that issue
add an option not to do structure tree work (per https://github.com/johnwhitington/cpdf-source/issues/301)
document it along with -collate

Have added -no-merge-struct-trees for now, and note to document specially with respect to -collate. Leaving open so we can fix merging of structure trees not to blow up.

Merging structure trees now not the default, so this is less urgent.
