Angular 2.0.0 supprt #30

This updates angulate2 to support the final 2.0.0 release of angular2 and 100% support for the tutorial heroes application at angular.io.

This mistakenly includes commits from https://github.com/jokade/angulate2/pull/26, https://github.com/jokade/angulate2/pull/27, and https://github.com/jokade/angulate2/pull/28. You can review/merge those first. I think that might be easiest. In fact, for full support of the heroes tutorial, those are needed anyway.

Thanks - I've refactored angulate2 for ES6 module support, hence this pull request is probably outdated.
