Add more places for custom formatting #21

This builds on #19 by adding more roles to the rst files.

Because of limitations in rst, some places can't have roles. Those are commented appropriately.

I had thought of making it so you could format each step name separately (i.e. gherkin-given-keyword, etc.), but then there is the question of what do you do with And, so I left it how it already is with just step in general.

Rebased and updated based on master. Has a new version number. Should look the same as it did before, and it is ready for final review
