Update for Home-Assistant after the 'Great Migration' #1

Home-Assistant recently changed the way they handle custom components, I've updated the structure of this component and added the necessary manifest files for the component to work again. I've also updated the readme accordingly and changed it to reflect the philips naming convention

Hi, sorry all, I am right in the middle of final year exam season at Uni and therefore have been off the pulse with Home Assistant for a while, what is this Great Migration? have your changes completely fixed the issue? I am quite happy to push your updates to the main repository if so. I am not currently able to keep on top of updates for this for at least a few weeks (I do however have some pretty cool new features I coded up for this component some months ago, I just need the time to implement them properly and at some point, push them out)

Hey, no worries! I'm in exam season myself right now so I understand you lol and yeah, they fixed the issues! At least the ones that were related to the migration. Basically the "Great Migration" is just a term the devs made up for the changes in custom components structure. :) Looking forward to those features!
