TOC links to practice problems #16

The links in the table of contents for the practice problems in each chapter instead link to the last section heading, instead of to the exercises. (This is typically a page or two before where it should link to.)

Figured it out. A /phantomsection in the stylesheet is the solution. (I'm currently making a number of changes and will push 2.0, including this fix, soon.)
