Subscribed users may show an exclamation mark in place of a profile image on the subscriptions list and saved posts tabs sometime after subscribing, likely as a result of a profile picture change.
The app may have cached an old image URL as clearing the app cache does not resolve the issue. Viewing the affected profile loads the new profile image but does not seem to replace the broken image on the subscription list.
