UpstreamPublisher changes #69

Let's make the UpstreamPublisher more robust.

It's worth noting that this acts as the "manager" side of things so its a two-way interface into the application...

let's do the following:

rename to UpstreamManager
add in prefix, hostname, and upstream name validation (uniqueness)
emit errors back to the plugins at write time
