Create internal survey #1

Create an internal survey, to gather some general info about the team. Since we don't expect more than 30 responses, feel free to use a few open questions.

Initial questions (need better phrasing):

• [ ] Are you on Windows, Mac or Linux? (OS/2 is dead. Get over it!)
• [ ] What Prototyping software do you use?
• [ ] Do you have a license for personal use? [conditional display if paid software is selected above]
• [ ] What other UX related software do you use?
• [ ] Do you have a license for personal use? [conditional display if paid software is selected above]
• [ ] Do you create prototypes on a regular basis?
• [ ] Do you create surveys on a regular basis?
• [ ] Do you do A/B testing on a regular basis?
• [ ] Do you do interviews on a regular basis?
• [ ] What is your experience with UX for mobile? [open question]
• [ ] What other skills can you bring to the Team? [need a list of relevant skills, such as design, programming, creative writing, infographics specialist, video editing, sound effects, voice actor/actress, etc.]

I think there needs to be a question regarding one's basic orientation: rational or intuitive. IE: Which approach best matches your approach to problem solving?
