Consumer Name Displayed with Health Declaration

As a business, I want to see the consumer's name along with there health declaration So that I can associate an identity with who is making the declaration and can verify it via driver's license, if needed.

We will agree it's done when the following acceptance criteria are met:

Acceptance Criteria:

• The consumer's name is within the home view.
• The health declaration is within the home view.
• The name displayed is the same as what the user entered.
