Add license #13

We should add a license to this project. I prefer an open source license. I like attribution as part of the license. I think the license must allow commercial usage. I like a share alike license. But it may be too restrictive for some parts of this spec. I like the CC-BY-SA license. It's what openEHR uses for archetypes. But it may not be a good fit for software.
