Add support for multiple teams #4

This really needs to be a feature as many people are now part of multiple Slack teams, however it will require a lot of time and refactoring to get this working properly.

Clicky was designed and built before multiple teams were commonplace, so most of the extension will need to be rewritten to support multiple sets of Slack data, as well as maintaining multiple websocket connections to enable notifications of incoming links as in #3
