Update "community" files #163

Update the "community" files - such as Issue Templates, Code of Conduct, Contributing guide - so that the developer experience is improved.

Of particular importance are improvements that help new contributors to get involved.

The code quality checks can safely be ignored for this PR as they relate to the use of "[" and "]" in the Issue template.

Hey @joshghent, I added these new files based on recommendations from GitHub for supporting new contributors to an open-source project. I also added a my name and "and contributors" to the license since there's a fe contributors now.

The Code of Conduct is based on an existing popular code. It's one of the ones GH offers in their code of conduct generator. The Code of Conduct requires an email for reporting issues (of contributors not following the Code; not bug issues). I added an email address of my own but I'd advise you change it to your own (git) email.

Since this is project management / contributing stuff, I'll leave this open for your approval.
