Browse by image and document does not take you to document or image in most cases

This related to an error I thought I caused when I upgraded WordPress like a month ago. All the URLs for pages broke, the hacktastic solution is found near the bottom of this page: http://wordpress.org/support/topic/permalinks-404-with-custom-post-type
