Since you asked to report how other banks use QR codes...

Crelan, one of the smaller Belgian banks, does not support QR codes for SEPA payments. Their iOS app opens the Payconiq app for scanning QR codes which, sadly, uses their own proprietary system as far as I can tell.

The Crelan mobile app now supports QR codes for ~~SEPA payments~~ following a recent update.

Update: Sorry, I was wrong, they only support Bancontact QR codes, not EPC QR codes.
