Missing form validations

Missing form validation on the following forms / fields:

Create appointment:

• start and end times, start must be before end
• counselor required
• school required
• participants required

Create school:

• phone format validation
• zip code value validation (numeric value)
• suggestion: make state field a select instead of free text

Create counselor:

• username (not sure of the expected format / length requirements)
• phone format validation

Create student:

• username (not sure of the expected format / length requirements)
• phone format validation
• docs URL (must be URL)

Create user:

• username (not sure of the expected format / length requirements)
• phone format validation

Expected behavior: When submitting the form with invalid data, users see validation messages for any field with invalid values.
