Add Student Form - Counselor and School Selections not cleared out #65

  1. Add a new student via the "Add Student" button and subsequent modal. Make note of your selections for assigned counselor and assigned school.
  2. Click the "Add Student" button again.

Result - The values of assigned counselor and school are saved from the prior entry. They should be cleared out instead.
