Host Manage Users Concept on Heroku #9

Wanted to make this easy for people to take for a test drive. Holding out until Node 6 is TLS before I can host node concepts on my VPS, so Heroku for now. Since package.json needs to be in the root, the Manage User concept is now/also in another repo at https://github.com/jpdevries/manage-users
