Bank account: Add account

Users should be able to add a bank/credit card account.

Capture;

• Account name
• Account number
• Sort code
• Notes
• Opening balance (date of opening balance, weather that figure is positive or negative)

When capturing credit card account, capture the card type & last 4 digits of card number
