Allow forwarding registration/icons with GNTP #386

Per issue 299:

Growl doesnt always forward registration/notification icons (may only be for 
apps/notifications that dont provide any and you guys are using the app's 
default icon - not sure yet)

I'm running into this problem when using the [gntp python package][1], and I'm 
not sure if this is a Growl error or the package. Can we get a little more 
specific as to what this is about?

I'm providing URLs to icons but Growl shows me the [Mac network icon][2] in the 
notification.

Quick addition to the previous comment: icons were working in 1.3.2, but 
stopped after I upgraded to 1.3.3.

URL based icons were broken on purpose, they were causing random CPU hangs, due 
to the blocking nature of the api we were using for downloading icons.  They 
will return in a future version, but Im afraid I can't be more precise on when 
right now.

Growl 2.0 will return support for URL based icons, as the entire stack was 
being rewritten, I took time to make sure it was multithreaded, and supported 
URL based icons.  Flagging as fixed in source.
