Disable self-service customer creation. #6

Indianapolis EMS Logistics requested that self-service customer creation from the main page and all other pages be disabled so internal and external customers need to contact them for account creation; credentials; county, agency, and unit assignment functions.

I confirmed that the fix was made correctly and that the customer account creation tab has been removed from the main login page.
