Would you mind adding a license to this so that we can know what permissions we have to copy and use it?

I want to included it with some notebooks that I'm submitting as supplementary material for an academic paper. Ideally you would copyright this as permissively as possible for ease of reuse.

No problem. In dbc8f434c385a0566f6a1eb219eb297df93f4ddb I've added a cc-by-3.0 license. Is that permissively enough for your purposes?
