chore(): Release automation

This PR adds various upgrades related to releases:

• updated dependencies & simplified things
• updates and fixes for npm > 5 & node.js 8
• added .npmignore to reduce the amount of clutter
• automated generation of changelog
• automated docs output
• simplified docs generation (markdown) and inclusions of docs on master & npm

I might still need to tweaks things down the line, but tests the best I could, and should make things simpler and less work :)
