Clean up `HW` and `FT` Columns #22

The Heartworm and FT (Flea/Tick) columns mostly have the dates of a treatment for that given medical condition. Easiest thing is to convert these to booleans for whether or not a dog has a date in the given column.

Note: FT is pretty bad data since it stopped being tracked in more recent dog sheets.

@rkelley05 check the name which may contain '(HW)' in it
