Specifying meta and snap device as the same device causes havoc. #163

What steps will reproduce the problem?

  Create a ddsnap volume, specifying the same device for the meta-device
and   the snapshot-device.
  Create snapshots and write data to the devices.

What is the expected output? What do you see instead?
  I expect the data to to be correctly isolated to snapshots, etc.
  Instead snapshot isolation is violated and partial crashes may occur.

Please use labels and text to provide additional information.
  The same operations specifying no meta-device works correctly.
