This adds a few more steps towards implementing #29

It does the following:

Adds a PR column with links to the pull request of each JEP
Updates the link for the title to link to the document for that JEP, if it exists
Updates the TOC in the Jupyter Book so that we separately show "Accepted" and "Implemented" JEPs
Updates some metadata to standardize our folder names
