This issue is meant to capture what goes on in a set of issues I'm now closing as its too hard to track things in multiple issues.
I think with https://github.com/jupyterhub/ldapauthenticator/pull/258 and getting https://github.com/jupyterhub/ldapauthenticator/issues/49 to resolve can make things work well again. If not, we should try to figure out what remains to resolve once and for all.
