I notice this thread is from 3 years ago, has anyone successfully installed with the new smart things app. There's been updates such as the smart app (below is not longer relevant). Is no one utilizing this switch anymore? Thanks in advance!

Configuring the SmartApp Now, grab your phone, and open up the SmartThings app. Touch the "Marketplace" icon in the lower right of the app -- it looks like an asterisk. Touch the "SmartApps" tab at the top of the page. Touch "My Apps", it's at the bottom of the list. Touch "Dual Relay Adapter". You'll be prompted with the question "Which Dual Relay Module?". Here, select whatever you named your relay (the one with two switches). For the "Virtual Switch to link to Switch 1?" and "Virtual Switch to link to Switch 2?" questions, just pick the virtual switches that we created above. Assign this instance of the SmartApp a name. Touch "Done"

I don't use the new app because there aren't routines and I don't want to re-build my scenes... But I just checked and the dual monoprice relay still seems to work to allow me to turn lights on and off with the new app. (Opening the relay doesn't work, but the switches themselves work just fine.) Not sure about adding the devices with the new app though, but most of the work is done on the website anyways.

I can't get this to work anymore. I first installed it Dec 2017. I replaced the relay in Dec 2018. A few months ago the virtual switches stopped functioning. I recently excluded and re-added the relay. I followed the above steps for linking virtual switches to the relay using the custom device handler. The virtual switches still don't work. Under my devices, if I tap the relay it switches one of the two circuits hooked to the relay on/off, so it looks like the relay is working.
