IRC server name changed. #522

Seems like the server name for IRC has been changed from irc.twitch.tv to irc.chat.twitch.tv. This should be changed in IRC.md.

irc.chat.twitch.tv is a temporary name during the transition. irc.twitch.tv will work after the migration is complete.
