hide label and placeholder in select options #189

Currently, we always include the label or placeholder in the select options as a disabled option. Some users (e.g. me) may not want this behavior. This PR allows a pair of configurables with default values to maintain compatibility with existing functionality that enable not including the label in the options of the Select box.
