There is a "description" field for each image which we aren't using (or showing to the user, currently).
Since all images are now inline, what about having a "caption style" which you simply place below each image, rather than creating a whole interface to input them separately from the main text?
Or, something like this (in Markdown syntax):
![caption goes here](/file/name.jpg)
Is the main desire that the caption text is styled distinctly and displayed close to/under the image?
The last two suggestions would make it easy to edit or add afterwards.
