ampersands get un-encoded

What steps will reproduce the problem?
1. Include an encode ampersand in the image caption field
2. Save
3. Edit the image again and save. The ampersand gets un-encoded.

What is the expected output? What do you see instead?

I would expect an encoded entity. 

What version of the product are you using? On what operating system?

txp v.4.5.4

Please provide any additional information below.

Not tested but all html entities might have the same problem
