TASK: create/edit alerts and allergies

Description:

// comprehensive allergy list

• list shows all allergies in db
• can change active status with toggle
• add button leads to create page where new allergies can be created
• edit button leads to edit page where existing allergies can be edited
• validation is implemented

// create

• forms need all fields from database tables (excluding PK and MHN)
• form fields need appropriate validation
• add button at top right of card -> goes to create form (see add button on all patients view) // edit
• edit button should be inline on table -> goes to edit form
• form should be pre-populated with alerts/allergies info (all fields)
• cancel and save buttons
• validation implemented
• 'are you sure?' message/pop up before save goes through
• when an alert is created, the patient's 'hasAlert' property needs to be set to true

Acceptance Criteria;

• all description items completed
• updated data must be saved to the database
• admin can access the create form via the 'add' button
• admin can access the edit form via the 'edit' button
• tested in dark mode and tablet view
