Inconsistencies in UI #107

What steps will reproduce the problem?
1. Navigate to Enter new Child form

Inconsistencies:
1.Diabetes Diagnosed should be displayed instead of Date Diabetes Diagnosed
2.Date insulin started is displayed instead of Insulin since

Expected Results---------
As per the excel sheet the above field labels should be
1. Diabetes Diagnosed
2. Insulin Since
