Centre name missing from drop-down #130

Hi Team,
Our colleague in Haridwar, India, has advised that the drop-down menu for 
"centre" comes up blank when entering new data.  I logged in as her (Santosh) 
but could not reproduce the issue i.e. it worked fine for me - asked her to try 
it again and she reported the same problem.  Could you please check?
