Add default payment processor selection to "No processor" #135

To activate payments we need to select a payment processor, but the form
("@@manage-getpaid-payment-options") gives the wrong idea that the testing
processor is already selected.

This could be solved with a new option ("None" or "No processor"), that
would be selected by default.
