Make elements in the checkout process configurable from the admin UI #182

There are a couple of things that would be great for store admins to be able to 
turn on/off in the 
checkout process:

Form stuff:
- the current mailer checkbox, which is hardcoded in
- the text format for the mailer, also hardcoded
- would be great to have a 'where did you hear about us?' element
- others?

Questionable things:
- whether or not to include the shipping page.  maybe this is better handled 
programatically?  just 
an idea :)
